If you are looking to book a course with Ocean Adventure's Ltd we are required to ensure each participant has printed out and completed a divers medical form, please ensure you have completed the correct form depending on whether you are completing a PADI or BSAC dive course.
This page is for the PADI divers medical form - If you require a BSAC medical form, please click the BSAC Medical Form tab.
If you are 18 years or older these forms must be completed yourself.
Participants under 18 years old must have these forms completed by a parent or guardian.
